Patricia Moran, DNP, APRN, ANP-C is full time faculty in the Adult- Gerontological Nurse Practitioner Health Program at Stony Brook. Dr. Moran completed her Doctor of Nursing Practice, a Post-Doctoral Certificate in Education and her Masters of Science in Adult Health as a Nurse Practitioner at Stony Brook University, a Bachelor of Science in Health Administration with a Certificates in Gerontology and Leadership and Human Resources from Saint Joseph’s College, an Associate of Applied Science in Nursing from Suffolk Community College and an Associate of Science in Dental Hygiene from Farmingdale State College. From 2013 to 2016 she was faculty on a grant titled “A Transformative Model in Interprofessional Education of the Care of Elderly Person with Multiple Chronic Conditions.” This grant allowed students from the Adult-Gerontology Nurse Practitioner program to work alongside the students in the Physician Assistant and Masters of Public Health programs in helping elderly patients with chronic medical conditions in a Team Based Learning environment. Her doctoral work was focused on reducing hospital readmissions for the acutely ill person with multiple medical conditions. She was successful in implementing the Society of Hospital Medicine's Project BOOST on a single medical floor in a large academia medical center that resulted in a readmission reduction of 4% in one quarter. Her Post-Doctoral project evaluated the use of a web conferencing software in distant learning with results showing high post course satisfaction for those who participated in the study. Dr. Moran is nationally certified as an Adult Nurse Practitioner with extensive experience as a Nurse Practitioner in the acute care setting. In addition, she was the coordinator for the Nurse Practitioner students at Long Island State Veteran's home facilitating an "at home" geriatric experience. She currently practices as an Adult Nurse Practitioner in a community hospital as well as volunteers with minorities and undocumented citizens who do not have access to medical care.

The purpose of this grant was to develop, implement, evaluate, and sustain interprofessional education with a focus on improving health outcomes for vulnerable and underserved elderly patients with multiple chronic conditions.
